A “Medical Home” for Patients

Partnerships between the patient, physicians, and their family are an integral part of medical care. That’s why PAHP provides each patient with a medical home, which is essentially an outcome-driven strategy for delivering the family-centered, comprehensive, continued and coordinated care all patients deserve.

The essential elements of a medical home include:

Personal Physician — Each patient has an ongoing relationship with a personal physician, trained to provide contact, continuous and comprehensive care.

Physician-Directed Medical Practice — The personal physician leads a team of individuals who
collectively take responsibility for the ongoing care of patients.

Whole-Person Orientation — The personal physician actively provides for the patient’s health care needs and takes responsibility for arranging the appropriate care with other qualified professionals.

Coordinated/Integrated Care — Under the Priority Access Health Plan, complex health care systems (specialists, hospitals, home health agencies, nursing homes, etc.) come together to coordinate and integrate services related to the patient’s primary care in a way that includes the patient’s loved ones and also incorporates community-based services.

Quality and Safety — PAHP is centered around providing compassionate, quality, patient-centered care rooted in evidence-based medicine through the use of decision-support tools. Physicians voluntarily engage in measuring performance in order to continually gauge and improve the quality of treatment and service they provide
